You can apply for the car wash insurance program in two ways.

Application for Self-Service Car Wash – (Customer Keeps Control of Vehicle)

Car Wash Workers' Compensation Coverage

You can apply for car wash workers' compensation coverage in two ways.

1. Download the car wash workers' compensation coverage application, fill it out and send it back to us via fax: 417-626-2923 or e-mail.

2. Submit your application online using our secured application Web site.

Please note: All brokers are required to submit a copy of their agency license and an Errors & Omissions Certificate for verification purposes. Once we bind coverage for your first submission, you will be mailed a broker kit with a Broker Agreement that you will need to complete, sign and mail, fax or email back to us.
