As promised, you now have the capability to submit a full-service car wash insurance application electronically!  To complete an application online, visit our Online Applications page and choose the Submit Online – Secured Site link, or simply click here.

If you prefer to complete an application as a hard copy and submit it via fax, email, or the postal service, you may do so by visiting our Online Application page and clicking on the Download PDF link for the appropriate application (Self-service/Exterior Only, Full-Service, or Workers’ Compensation).  The PDF format allows you to enter the information on your computer however, it will not be saved.  Be sure to print the completed application, sign it, and send it to The Insurancenter, 2901 Arizona Avenue, Joplin, MO 64804-3180; fax to 417-626-2923, or email to carwash@carwashinsurance.com.  Don’t forget to keep a copy for your records as well.
